The diagnosis is gangrenous sigmoid volvulus. Sigmoid volvulus commonly occurs in older adults with a history of constipation. Endoscopic detorsion with rectal tube decompression is successful in most instances. However, emergency surgery is indicated for patients with signs of peritonitis or bowel ischaemia.
